Strategy Writeup

Once you've completed the previous objectives and enter the throne room, The Countess will spawn in the center. She will attack the first player to aggro her. The strategy here is one person should be the aggro and the rest of the players should be attacking her from behind or from a distance. She has an attack where she lifts up her sword with two hands and charges it, and a red wide cone will be marked on the ground indicating the the direction where she'll swing the sword. Move away from the area as she will slash that area causing heavy AoE damage.

Once you've managed to take away her first health bar, she will disappear and summon Bloodsworn enemies in the area to help fight alongside her. At this phase, watch out for a red circle on the ground, this indicates where The Countess will land and slam on the ground causing AoE damage. She also has a move where she'll cast a red line on the ground, standing on this line will cause continuous damage until you move away from it, the best way to avoid this is to never stand in the direction where The Countess is facing as she can instantly cast this red line from time to time. The red line will stay on the ground until The Countess is defeated. The Countess will also fly back to the sky and land immediately in the center of the room, a swarm of blood bats will then emerge that shoots outwards throughout the room. More Bloodsworn enemies can be summoned at this point, stay away from Bloodsworn Oppressors and defeat the spawned enemies quickly, the Oppressors can knock you back into the red line or to the blood bats roaming around the room. Taking care of the Bloodsworn enemies immediately should help you and your party focus on evading the Blood Bats and avoiding the red lines on the ground while focusing all your damage input to The Countess.

After defeating The Countess, all 5 Bonus Treasure Chests will appear in the middle of the room considering you've completed all the previous objectives beforehand. The Countess has a chance to drop Legendary Item(s) and she can also drop Set Items in Hell 1+ difficulty.

Attacks & Counters

Attack Counter
Charged Slash: Lifts up her sword and charges it, then swings in a wide area causing heavy AoE damage. Keep an eye out when you see a red cone on the ground, this indicates the direction where The Countess will swing her sword. You should have about 2-3 seconds to move away from the area before she swings her sword.
Bloodsworn Summons: The Countess flies into the air and summons 9 Bloodsworn Enemies to fight alongside her. Defeat all the Bloodsworn enemies by grouping them together and using a heavy AoE attack to quickly eliminate them.
Landing Strike: The Countess flies into the air then slams on the ground dealing AoE damage. A red circle will appear on the ground indicating the area where The Countess will drop and strike the ground to deal AoE damage. This move will likely happen while you're trying to fight the Bloodsworn Enemies she's summoned.
Blood Bats Swarm: The Countess flies into the air and land immediately in the center and summon a swarm of Blood Bats that flies throughout the room. As The Countess channels for this move, try to move away from her and find your positioning where you can maneuver easily and stay away from the Blood Bats flying outwards. These Blood Bats will keep flying around the arena.
Bloodflame: The Countess conjures a red line on the ground that will cause continuous damage to a player as long as they're standing on it. Avoid standing in the direction where The Countess is facing as she can immediately cast this spell instantly. The red lines on the ground will stay in the arena until The Countess is defeated.

The Countess Diablo Immortal Lore

And so it came to pass that the Countess, who once bathed in the rejuvenating blood of a hundred virgins, was buried alive... And her castle in which so many cruel deeds took place fell rapidly into ruin. Rising over the buried dungeons in that god-forsaken wilderness, a solitary tower, like some monument to Evil, is all that remains.
